LTC@Home is your partner in managing the specialized, changing needs of those suffering from chronic health conditions or who have special care needs. We ensure that our caregivers are trained in the specific disease processes or condition our clients have, whether it’s ALS, cancer, diabetes, chronic lung disease (COPD), MS, Parkinson’s disease, stroke or some other disease or disorder.

In addition to understand client needs for specific conditions, our caregivers will take the worry out of daily activities. We can help maintain a safe environment by assisting in getting the necessary medical equipment or home modifications. We provide help with errands and housekeeping to allow you to live as independently as possible. Our caregivers will be there if mobility becomes difficult or if cooking for yourself is no longer possible. We can properly and safely help you get to the bathroom or move from your bed to a chair. We do whatever is needed to help our clients maintain physical and emotional health.
